Please note that EAC will generally only reimburse up to $500 on the cost of a refrigerator. Installation and hook-ups (e.g., water) must be coordinated through and approved by the Facilities Management Department.

Use this form to request small appliances and financial assistance for events that promote team building and employee morale. All requests must be approved in advance.

Use this form to request reimbursement for an approved expense.
